SIGNATURES

By signing below registrant understands that any misconduct may result in elimination from the entire convention. Furthermore, attending orientation on Thursday is mandatory in order to receive a convention badge & T-shirt, which must be worn at all times for admittance into religious and sports activities. Only after attending all religious programs will registrant receive a pass to the Saturday night banquet.

MANDATORY INFORMATION FOR EVERYONE

 

The estimated registration fee for this event is $250.  This fee will cover transportation from San Diego to San Jose and back. Also included will be room and board for 3 nights and 4 days.  Food and beverages will be provided by the hosting church, St. Mary’s.  If you need to cancel your reservation after you have registered, you must notify the church as soon as possible. If you cancel on June 5th, 2008 or later, we regret to inform you that we will not be able to refund your registration fee of $250.  As our fundraising efforts persist, the $250 registration fee will be reduced & will be announced by the end of May.

SPORTS DISCLAIMER

SPORTSMANLIKE CONDUCT

All participants are expected to abide by the stated policy, and shall conduct themselves in a sportsmanlike and suitable behavior in connection with sporting events, and related functions in accordance with the standards herein. In addition, each participated member should fully participate in all scheduled activities.  Failure to appear will result in elimination from attending this event.

We realize that the reason of this agreement is to ensure the safety of all the participants at the Convention. This is to ensure conduct and actions that will result in each participant receiving the full benefits and enjoyment of the enlightening experience from this event.

DEADLINE

In order to participate in this tournament, this form must be received no later than May 15th 2008.  NO TEAMS WILL BE ALLOWED TO PARTICIPATE IF THEY HAVE NOT REGISTERED BY THIS DATE.  Hurry and register, as there is a limit on the total number of teams allowed to participate.

Neither St. Mary’s Church nor its affiliated locations are responsible for any injuries that may result. 2008 Youth Pilgrimage

(Concluding with the Diocesan Youth Convention)
